Catalog and Curriculum Development
- Course Proposal - For use by faculty to propose new courses for the Catalog, General Education, or U. S. Cultural Pluralism.
- Course Modification For use by faculty to propose changes to existing courses.
- eLearning Addendum - Append to a new Course Proposal or Course Modification Request form for any new or existing course in which a total of more than 50% of traditional face-to-face instruction time is to be replaced with eLearning technologies.
- Selected Topic Course - For use by faculty to propose a selected topic course (270, 470, 471, 570, 571, etc.)

Consultation Memos to Other Departments- For use by one department to notify other department(s) of proposed course changes and/or changes to curriculum requirements that affect the other department(s).
- Course Change- memo to notify another department of a change to a course that is included in their curriculum.
- New Course- memo to notify another department of a new course that may have content that overlaps with a course in their department.
- Requisite Change- memo to notify another department that one of their courses is being added or removed as a requisite for a course.
- Curriculum Change- memo to notify another department that one of their courses is being added or removed as a requirement in a program’s curriculum.
- Blanket Curriculum Substitution - For use by Department Chair/Head to propose a curriculum substitution; the change applies to all students in the major.
- Add Request for Approved Electives - For use by Department Chair/Head to add a course to a list of approved electives in an existing catalog for a program or concentration.
- Concentration Proposal Outline – For use by faculty to propose a new concentration for an undergraduate program.
- Specialization - For use by faculty to propose a new specialization for a graduate program.
- Minor Proposal Outline – For use by faculty to propose a new minor.
- Graduate Certificate Program - For use by faculty to propose a new graduate certificate program.
